To register your child for an upcoming dental clinic at their school, please complete and submit the registration form below.
Ready, Set, Smile provides all services at no cost to families.
If your child has insurance, we ask that you text (612-887-3555) a picture of the front and back of the insurance card to us. We bill insurance companies directly, and families are never charged. If an insurance claim is denied for any reason, Ready, Set, Smile will cover the cost.
